Short explanation of differences/similarities between Branded and Generic medications. Branded drugs are essentially the first-to-market medicines and are covered in the initial period of marketing by a patent. They tend to be priced high because of the need to recoup investments made during research and development. Post licensing, the branded drug enjoys a protect period while being covered by patent. Once the patent runs out, the active ingredient can be marketed in generic forms. Generic drugs appear in the market once the patent cover for the drug expires. They are licensed primarily based on adherence to quality manufacturing standards, and the establishment of therapeutic 'equivalence' or bio-equivalence. Essentially generic drugs must in interchangeable with the branded item. The regulation for this is usually the national drug regulatory departments. In Singapore, this would be handled by the Health Sciences Authority (HSA) Singapore. Generic drugs are priced much lower because companies marketing generic drugs have not made any prior R&D investments during drug development. Generally, generic drugs are as safe and effective as branded ones, but much cheaper. Unless really necessary, prescribers should use generic medicines. From Boston -- Over 1/3 of US physicians are prescribing brand-name drugs at the patient's request instead of generic equivalents, according to a report published in JAMA Internal Medicine. Researchers surveyed nearly 1,900 US physicians from several disciplines. They found that 37% of physicians reported prescribing brand-name medication at a patients request. As well, those who were in practice for over 30 years were most likely to prescribe a brand-name medication because a patient requested it, despite the presence of generic equivalent.

THE HEALTHCARE ACCESS PROBLEM: In developed countries, once the patent of an innovator drug expires, 90% of consumers shift to generics but in developing countries like the Philippines, the opposite is observed – 90% patients remain loyal with branded drugs! Worse, most affected Filipinos are those from the poorest quintile (~20 Million). More than 65% of their out-of-pocket expenses were medicine purchases (2014). In a survey of 1160 Filipinos (2014), 47% were faithful to branded prescription and the remaining did not know that they could ask for the prices of the generic alternative. As a consequence, many of them suffer complications and die. In fact, 3 of 30 customers are able to sustain maintenance drugs for a year due to high cost. Why does the problem exist? Filipinos lack knowledge on generics (~50%), believe that generics are inferior (49%) and pharmaceutical companies focus on strong marketing efforts to physicians who are key influencers of purchase. As long as doctors continue to prescribe branded drugs only without writing generic alternatives, more Filipinos would suffer complications and even die. If only they have access to the right information then thousands of lives could be saved! THE PROPOSED M-HEALTH SOLUTION: GeneRx (pronounced as Gé-né-ríx) is the “google” and “ebay” for generic drugs in the Philippines as it offers the value proposition of “going generics, made easy.” GeneRx solves the pain points of buying expensive branded drugs and inadequate information on alternatives. “Going generics” is “made easy” by making information accessible at the patients’ fingertips. After downloading the free mobile app, users can just enter the brand name of prescribed drugs and GeneRx will revert with information about generic alternatives, equivalent doses, manufacturer, price, and availability in local drug stores. If the brand name is a combination of two drugs and combination generics is not available, GeneRx will give users generic alternatives of separate drugs with equivalent dosing recommendation. For example, if a prescription reads “Exforge 5/80mg,” a market leader anti-hypertension drugs by Novartis, GeneRx will revert with its generic equivalents such as combination “Amlodipine/Valsartan 5/80mg” or separate “Amlodipine 5mg” and “Valsartan 80mg” depending on local market availability. To increase user engagement and trust for generics, additional information about the drug (mode of action and side effects) and ratings (submitted by healthcare professionals and previous users) shall be posted. Results can be sorted according to price, nearby location, and ratings. Users can even opt to order online and have these medications delivered right at comfort of their own houses! Based from our validation, 94% will try the app and all 13 indigent patients interviewed expressed strong interest! GEOGRAPHICAL FOCUS: To ensure success of the project, it will be employed in phases with each phase building on the learnings of the previous one as the it is scaled up. Through this strategy, each level shall be self-correcting (mistakes are discovered early and corrected) and inspirational (feedbacks from smaller lead users inspire one change after the other) until the prototype is optimized before it reaches nationwide roll-out. "Made in China" is something that's seen in a lot of products, and most people are okay with it. It sometimes carry a negative connotation, and really depends on the product. For cheaper and lower tech items it's not that big of a deal. Even things like generic action cameras and other mainstream electronics from China are generally accepted and sometimes quite popular. But what about higher end PC components? Like the GPU? I got a chance to check out a GPU from Yeston, a brand based in China I've never heard of. There is virtually no coverage of any of these cards online. Note, these are still Nvidia GPUs, the manufactures just equip it with custom coolers, PCBs, clock profiles, etc. I tested the Chinese card and compared it to an equivalent card I've used from one of the big US based brands. Definition: The Property within two drugs with identical activity ingredients or two different dosage forms of the same drug process, similar bio-availability and produce the same effect at the site of physiological activity. FDA definition: The United States Food and Drug Administration (FDA) has defined bioequivalence as, "the absence of a significant difference in the rate and extent to which the active ingredient or active moiety in pharmaceutical equivalents or pharmaceutical alternatives becomes available at the site of drug action when administered at the same molar dose under similar conditions in an appropriately designed study.
